The ODBA affiliation fee for 2008-09 is $3 per member. Note that under the ODBA’s By-Law No.1, a Club must have at least 10 members in order to affiliate.

 

Please complete this form, photocopy for your records, and send, with your fee, to:

ODBA Treasurer, Terry Leung, 64 Bateman Drive, Nepean, ON K2G 5H5.  A copy will be forwarded to the OBA to complete your affiliation. 

 

Please note:  This form is used to update the OBA web site - Where to Play - and to provide a breakdown of membership for the grant application to the Ontario government that helps to support all Ontario badminton.
